Yes. Really sad. I recall that it even made the nightly news back in the US. Forgot exactly what happened, but I think it had something to do with the decocker mechanism on the M9 Beretta. If the hammer is cocked and you place the pistol on safe the pistol is suppose to decock safely. If I recall that did not happen and the pistol went off.
