UMN soil testing lab tests for orchards. I can vouch for their quality as a lab. I've run a few thousand samples through them. Probably one of the more reasonably priced too.

Thanks MO extension also does it I found today and a couple in Kansas

I sent the samples off to MO extension at the University and got back yesterday. They seem very well done and I will fertilize in spring.
