Can anyone tell me what has happened to a few of my grafted trees? The bark seems to be turning dark brown and the leaves are beginning to die/wilt. Thanks

Looks like the rootstock died. Do you think you overwatered and the roots rotted?

I didn't think I did, but I suppose I could have. The soil is about 1/3 potting soil, 1/3 Perlite and 1/3 pine bark with some grass clippings on the top to help retain moisture because usually it dries out really fast. Which is the same mix I use for chestnuts. Could the grass clippings be the problem? There is a nursery not to far from me and when I bought some persimmon seedlings from them they had a small amount of clippings on top, the owner said it is just to retain moisture. What is a good soil or mix for newly grafted apple/pear trees?

They did come from my lawn, but it hasn't been sprayed since last year. Out of roughly 40 trees in the exact same conditions it seems to only be affecting 3-4. If it was the grass clippings do you think they would all be affected?

It's almost certainly not from herbicide applied to the lawn last year. Generally, a blade of grass only has a life span of 40-60 days.

Ill check that off the list. In the last picture you can see the bud turning brown, seems odd it would be all green around it and yet the bud is turning brown. I have been removing any growth below the graft if it has taken, not sure if an open wound from removing growth would invite any issues?
