Some poor graft results from last year

I did 8 grafts last year on G.41 dwarf rootstock. The Geneva's don't always have great roots out of the stoolbed and what I got had some that were better and some not. I wish I had photographed them before I planted. One rootstock didn't have a single root and I just planted it in hopes that maybe it would root and be graftable later. It got a tiny leaf but died by the end of the summer. I had 3 others where the scion leafed out and grew an inch or so. The other 4 grafts healed nicely and grew well.

I decided to move them all into pots in hopes of planting out later this year and I wanted space in my nursery for this year's grafts. The G41 that grew well had nice roots and I had to trim some to get them into 5 gal pots. I was hoping the 3 that did not grow much would get going this year but I found they had little to no roots left. No wonder they never did anything last summer. I ended up tossing them. I really wish I had the photos to compare roots as received and as tossed.
 
My results are not looking good this year. I had scions left over after field grafting and got a half dozen M111. I grafted them with my left over grin scions in mid March (indoors in 1 gal RB2s). All of the M111 have buds that are leafing out but I'm seeing no bud swell or greening on the grafts. I keep pinching off the leaves up near the graft so they don't out-compete the graft but I'm leaving the lower ones to keep the M111 going in case I need a second try.

Most of my G30 roots were decent from cummins, I didn't lose any of the rootstock the only 2 that put on serious size were Pinova & Pink Pearl (around 3-4 ft each) that I planted at my parents otherwise the most they grew in my nursery were like 10 inches.
 
I just got some g.969 from Cummins and they had great roots - some of the best looking root stock that I've ever gotten.
 
I saved a G41 that had a single root on it. The scion hadn't done much and I broke if off messing around with it. But there was a small piece of scion still in the cleft. I wrapped that one up and stuck in the fridge to maybe graft later. I looked at it tonight and see that little piece of scion in the cleft has a bud that is pushing. So I might just replant it but I'm not sure what the variety is now, either Goldrush or PRI 1918-1. So it may have taken off this year if I had left in the ground another year. I'll put it into a pot without grafting it and see how it does.
 
I'm scratching my head trying to figure out if I did something wrong. Perhaps I grafted too early on the field crabs I grafted. They had leaves on them and some even blossoms but we got a cold snap in the 20s after I grafted that set them back. However, at about the same time, I grafted some on M111 and put them indoors under lights. The M111 buds are all leafed out. I keep pinching off growth except for one well below the graft that I want to keep to ensure the M111 stays alive if the grafts fail. None of these show any signs of bud swell yet.

So, it is either the scions (all came from GRIN and were smaller diameter than I've used before), or it is my poor grafting technique. I'm guessing the latter!

Just an update... I think I did graft too early but I'm finally starting to see a few of the field grafts succeed. I have 2 with leaves, 1 with a swollen green bud, and a couple with not bud swell but show signs of callus. I just did some maintenance on them all today. Several of them clearly did not take and I regrafted them today.

So, I can understand how grafting and then having a few very cold nights can be problematic, but the interesting thing here is the grafts on M111 done inside and planted in RB2s under lights all failed. I can't explain how my field grafting worked out somewhat better than my bench grafting on m111.
